ABSTRACT

Objective To investigate the clinical value of serum metabolomic profile of prostate cancer using nuclear magnetic resonance-based metabolomics.Methods The retrospective case control study was adopted.The clinical data of 31 patients with prostate cancer,28 patients of prostatic hyperplasia and 31 healthy volunteers were enrolled in this study from May 2016 to May 2017 at the first affiliated hospital of Xinjiang medical university.In PCa group,the mean age was 66.3 years old,ranging 53-80 years old.In BPH group,the mean age was 59.3 years old,ranging 46-75 years old.In volunteer group,the mean age was 47.8 years old,ranging 35-62 years old..The serum of the 3 groups was measured by 1H-NMR spectroscopy.Multivariate statistical analysis was used to analyze the serum differential metabolism of the 3 groups,including principal components analysis (PCA),partial least squares discriminant analysis (PLS-DA) and orthogonal partial least squares discriminant analysis (OPLS-DA).Results The multivariate statistical analysis of PCA that the rate of the first principal component 1 (PC1) was 53.24%,the second principal component 2 (PC2) was 25.31% and the cumulative contribution rate was 78.55 %.Results of PLS -DA showed that partial data overlap of the three groups,but the separation trend was appeared.The variance of X(R2X) and Y(R2Y) matrixes and predictive value Q2 were 0.67,0.60,and 0.42.The results of OPLS-DA showed that the difference among the PCa group and BPH group,healthy group were obvious.The separation trend were appeared and the differential metabolites could be screened effectively.The R2X、R2Y and Q2 was 0.24,0.57,0.21 and 0.30,0.65,0.36.26 different serum metabolites were detected in the 3 groups,including citric acid,arginine,threonine,citrulline,glutamine,lactic acid,alanine,unsaturated fats,glycoprotein etc.Conclusions Compared with BPH group and healthy group,the serum of prostate cancer patients showed significant differences in metabolism.Nuclear magnetic resonance metabolomics analysis can effectively distinguish these serum metabolic differences.

ABSTRACT

Objective@#To investigate the clinical effect and safety of long-acting pegylated interferon-α-2b (Peg-IFN-α-2b) (Y shape, 40 kD) injection (180 μg/week) in the treatment of HBeAg-positive chronic hepatitis B (CHB) patients, with standard-dose Peg-IFN-α-2a as positive control.@*Methods@#This study was a multicenter, randomized, open-label, and positive-controlled phase III clinical trial. Eligible HBeAg-positive CHB patients were screened out and randomized to Peg-IFN-α-2b (Y shape, 40 kD) trial group and Peg-IFN-α-2a control group at a ratio of 2:1. The course of treatment was 48 weeks and the patients were followed up for 24 weeks after drug withdrawal. Plasma samples were collected at screening, baseline, and 12, 24, 36, 48, 60, and 72 weeks for centralized detection. COBAS® Ampliprep/COBAS® TaqMan® HBV Test was used to measure HBV DNA level by quantitative real-time PCR. Electrochemiluminescence immunoassay with Elecsys kit was used to measure HBV markers (HBsAg, anti-HBs, HBeAg, anti-HBe). Adverse events were recorded in detail. The primary outcome measure was HBeAg seroconversion rate after the 24-week follow-up, and non-inferiority was also tested. The difference in HBeAg seroconversion rate after treatment between the trial group and the control group and two-sided confidence interval (CI) were calculated, and non-inferiority was demonstrated if the lower limit of 95% CI was > -10%. The t-test, chi-square test, or rank sum test was used according to the types and features of data.@*Results@#A total of 855 HBeAg-positive CHB patients were enrolled and 820 of them received treatment (538 in the trial group and 282 in the control group). The data of the full analysis set showed that HBeAg seroconversion rate at week 72 was 27.32% in the trial group and 22.70% in the control group with a rate difference of 4.63% (95% CI -1.54% to 10.80%, P = 0.1493). The data of the per-protocol set showed that HBeAg seroconversion rate at week 72 was 30.75% in the trial group and 27.14% in the control group with a rate difference of 3.61% (95% CI -3.87% to 11.09%, P = 0.3436). 95% CI met the non-inferiority criteria, and the trial group was non-inferior to the control group. The two groups had similar incidence rates of adverse events, serious adverse events, and common adverse events.@*Conclusion@#In Peg-IFN-α regimen for HBeAg-positive CHB patients, the new drug Peg-IFN-α-2b (Y shape, 40 kD) has comparable effect and safety to the control drug Peg-IFN-α-2a.

ABSTRACT

Objective@#To investigate the efficacy and safety of the new investigational drug pegylated interferon α-2b (Peg-IFN-α-2b) (Y shape, 40 kD) injection (180 µg/week) combined with ribavirin in the treatment of patients with genotype 1/6 chronic hepatitis C (CHC), with standard-dose Peg-IFN-α-2a combined with ribavirin as a positive control.@*Methods@#A multicenter, randomized, open-label, and positive-controlled phase III clinical trial was performed. Eligible patients with genotype 1/6 CHC were screened out and randomly divided into Peg-IFN-α-2b(Y shape, 40kD) group and Peg-IFN-α-2a group at a ratio of 2:1. The patients in both groups were given oral ribavirin for 48 weeks in addition and then followed up for 24 weeks after drug withdrawal. Abbott Real Time HCV Genotype II was used to determine HCV genotype, and Cobas TaqMan quantitative real-time PCR was used to measure HCV RNA level at 0, 4, 12, 24, 48, and 72 weeks. Adverse events were recorded in detail. The primary efficacy endpoint was sustained virological response (SVR), and a non-inferiority test was also performed.@*Results@#A total of 561 patients with genotype 1/6 CHC were enrolled, among whom 529 received treatment; 90.9% of these patients had genotype 1 CHC. The data of the full analysis set showed that SVR rate was 69.80% (95% CI 65.00%-74.60%) in the trial group and 74.16% (95% CI 67.73%-80.59%) in the control group (P = 0.297 0). The data of the per protocol set (PPS) showed that SVR rate was 80.63% (95% CI 76.04%-85.23%) in the trial group and 81.33% (95% CI 75.10%-87.57%) in the control group (P = 0.849 8), and the 95% CI of rate difference conformed to the non-inferiority standard. The analysis of the PPS population showed that of all subjects, 47.9% achieved rapid virologic response, with a positive predictive value of 93.8%. The incidence rate of adverse events was 96.30% in the trial group and 94.94% in the control group, and the incidence rate of serious adverse events was 5.13% in the trail group and 5.06% in the control group.@*Conclusion@#In the regimen of Peg-IFN-α combined with ribavirin for the treatment of genotype 1/6 CHC, the new investigational drug Peg-IFN-α-2b(Y shape, 40 kD) has comparable clinical effect and safety to the control drug Peg-IFN-α-2a.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical characteristics of an infant with neonatal diabetes mellitus (NDM) and to sequence the ABCC8 gene of this family in order to provide a theoretical basis for the diagnosis and treatment.</p><p><b>METHODS</b>The clinical data of the patient was collected, and the proband and his direct relatives within three generations were sequenced.</p><p><b>RESULTS</b>The patient was 1-month-old, random blood glucose was more than 27.8 mmol/L, C-peptide was 33.8 pmol/L, blood gas analysis was pH 7.16, HCO3.9 mmol/L and urine alkone was 3+. Genetic testing revealed that the patient, his father, elder brother and grandmother have carried heterozygous mutation c.2690A>T(p.D897V) of the ABCC8 gene. Fluid infusion, intravenous administration of insulin and other supportive therapies were provided. After the correction of acidosis, subcutaneous insulin injection were uesd to control the blood glucose. Eight months later, blood glucose was pooly controlled. After combined with glibenclamide, blood glucose was under control.</p><p><b>CONCLUSION</b>The patient carries a heterozygous mutation c.2690A>T(p.D897V) of ABCC8 gene, which is a novel mutation. Glibenclamide was partly effective for the patient.</p>

ABSTRACT

ObjectiveTo explore the role of activation and expression of absent in melanoma 2 (AIM2) in pathogenesis of hepatitis B virus (HBV) associated glomerulonephritis (HBV-GN).MethodsFifty-four patients with HBV-GN were recruited into the investigational group,and 25 patients with chronic glomerulonephritis (CGN) and 6 patients with chronic hepatitis B (CHB) were included as negative control group and positive control group,respectively.Immunohistochemistry was used to detect AIM2,Caspase-1 and interleukin (IL)-1β expressions in both kidney tissues from the investigational group and negative control group and liver tissues from positive control group.The differences between groups were compared by chi-square test,and the correlation analysis was conducted by Spearman correlation test.ResultsThe positive expression rates of AIM2 in 54 patients of HBV-GN group and 25 of CGN group were 81.4 % and 4.0%,respectively (x2 =38.746,P< 0.01).AIM2 was found to be expressed in all 6 CHB patients.In HBV-GN group,the expression of AIM2 was positively correlated with Caspase-1 (rs =0.444,P<0.01),and the expression of IL-1β was also positively correlated with Caspase-1 (rs =0.515,P<0.01 ).In HBV-GN group,the expression of AIM2 in patients with HBV DNA≥1 ×105 copy/mL was significantly higher than that in those with HBV DNA<1 × 105 copy/mL (x2 =6.097,P<0.05).ConclusionThe activation of AIM2 may play a role in the pathogenesis of HBV-GN through Caspase-1 activation and inflammatory factor IL-1β release.

ABSTRACT

Objective To explore the impact of nucleos(t)ide analogue antiviral therapy on anxiety and depression of patients with chronic hepatitis B (CHB),and analyze the related factors.Methods Before nucleos(t)ide analogue antiviral therapy,1 year and 2 years after antiviral therapy,120 CHB patients were investigated with self-rating anxiety scale (SAS) and self-rating depression scale (SDS).The demography data of patients were collected.Serum levels of alanine transaminase (ALT) and hepatitis B virus (HBV) DNA and other biochemical indicators were measured regularly.Results Before nucleos(t)ide analogue antiviral therapy,1 year and 2 years after antiviral therapy,both the mean scores of SAS and SDS became lower gradually (F=12.661 and 22.395,respectively;both P<0.01).The percentage of patients with SAS and SDS scores more than 50 were 5.8%,4.2%,1.7% and 13.3%,7.5%,5.0%,respectively.After 2 years of therapy,the anxiety improvement rate of the patients obtained HBV DNA<1000 copy/mL was 69.0%,while those with HBV DNA≥1000 copy/mL was 22.2% (x2 =22.325,P<0.01).Meanwhile,after 2 years of therapy,the depression improvement rate of the patients obtained HBV DNA<1000 copy/mL was 77.4%,while those with HBV DNA≥1000 copy/mL was 22.2% (x2 =32.179,P<0.01).Multiple factors Logistic regression analysis indicated that the odds ratios (OR) of improvement of anxiety and depression in patients with HBV DNA<1000 copy/mL were 7.751 (95% CI:3.026-19.853) and 15.069(95% CI:5.309-42.770),respectively,compared with those with HBV DNA≥1000 copy/mL; and OR of improvement of depression in patients with ALT≤40 U/L waa 4.103 (95% CI: 1.376 - 12.238).Conclusions Nucleos(t) ide analogue antiviral therapy could improve the anxiety and depression of CHB patients.The HBV DNA negativity is the independent impact factor of improvement of anxiety and depression in CHB the patients.

ABSTRACT

Objective To evaluate the effect of the fast control scheme of schistosomiasis.Methods The fast control scheme of schistosomiasis was carried out,and the scheme mainly included dusting molluscicide,rapid immunodiagnosis screening and chemotherapy with praziquantel,environment modification,health education,etc.The control effects of the scheme were observed longitudinally.Results There were no infected snails found after the scheme was implemented from 2005 to 2008.The densities of living snails dropped from 1.48 snails/0.1 m~2 in 2004 to 0.71 snails/0.1 m~2 in 2008.The areas with snails dropped from 43.13 hm~2 in 2004 to 33.68 hm~2 in 2008.The infection rates of residents and cattle dropped to 0 in 2008.Conclusion The fast control scheme of schistosomiasis can control schistosomiasis fast.

ABSTRACT

Objective To study the expression of phosphorylated serine 910 of focal adhesion kinase(FAKps910)in circulation endothelial cells(CEC)from patients with hemorrhagic fever with renal syndrome(HFRS).Methods Fifty HFRS patients were involved in the study and divided into mild(n=17),moderate(n=20)and severe(n=13)groups according to patients' conditions. Twenty healthy volunteers were enrolled as control group. CEC were isolated by Percoll density gradient centrifugation method. The expression of FAKps910 in specific APC-CD31+ CEC was measured by flow cytometry. The data were analyzed by one-way ANOVA. Results The positive rates of FAKps910 in CEC were 59.87%±9.58% in early febrile stage patients and 21.14%±2.53% in the healthy controls, while it was 11.64%±2.17%in the later febrile or hypotension stage patients(F=262.31,P<0.01).It gradually restored to normal afterwards. The positive rates of FAKps910 in CEC were 17.45%±2.64%,13.84%±2.54% and 7.47%±2.57%,respectively in mild, morderate and severe groups in the later febrile or hypotension stage, which were all significantly different from that in control group(F=52.642,P<0.01).Conclusions FAK in CEC iS related with the clinical severity of HFRS patients. FAK may be involved in intergrin β3-mediated endothelial injury during Hantavirus infection.

ABSTRACT

Objective To explore the relationship between intrahepatic expression of interleukin-17 (IL-17) and liver fibrosis in patients with chronic hepatitis B virus infection. Methods IL-17 expressions in livers with different inflammation activity grades and hepatic fibrosis stages from patients with chronic hepatitis B virus carriers (n= 30), chronic hepatitis B (CHB, n = 55), liver cirrhosis (LC, n=20) were measured by immunohistochemistry. Serum IL-17 and liver fibrosis indices of haluronic acid (HA), laminin (LN), type Ⅲ procollagen (PC Ⅲ ) and type Ⅳ collagen ( Ⅳ C) were determined by enzyme linked immunosorbent assay (ELISA). The differences between groups were compared by Kruskal-Wallis test, and the Mann-Whitney test, and the correlation analysis was done by Spearman test. Results Intrahepatic IL-17 expression in LC group was significantly higher than CHB group (x2 =25. 3982, P=0. 004), and that in CHB group was higher than chronic hepatitis B virus carriers group (x2 = 11. 5056, P= 0. 001). The inflammation activity grade and hepatic fibrosis stage were both positively correlated with IL-17 expression (r= 0.718, 0. 693, respectively; both P<0.01). IL-17 mainly located in portal area and the expression was positively correlated with serum levels of HA, LN, PCⅢ and ⅣC (r=0. 793, 0. 834, 0. 722, 0. 883, respectively; all P<0.01).Conclusion Intrahepatic IL-17 expression is closely correlated with liver inflammation activity grade and hepatic fibrosis stage.

ABSTRACT

Objective To explore the expression of interferon(IFN)-γ,interleukin(IL)-4 and IL-17 in the patients with hepatitis B virus(HBV)infection,which can reflect the function of helDer T lymphocyte(Th)1,Th2 and Th17 cells.Methods There were 34 chronic hepatitis B(CHB)Datients,31 liver cirrhosis(LC)patients,26 primary hepatocellular carcinoma(PHC)patients,22 chronic severe hepatitis patients and 15 healthy controls involved in our research.The levels of IFN-γ,IL-4 and IL-17 in serum of the patients with chronic HBV infection and healthy controls were determined by enzyme linked immunosorbent assays(ELISA). And the dala were analysed via t-test and Pearson correlation analysis.Results IFN-γ/IL-4 ratio of the CHB group(1.08±0.66,P<0.01)was lower than that of healthy controls(2.60±0.60),and the ratio in the chronic severe hepatitis B group(4.81±0.87,P<0.01)was higher than that in healthy control group.IFN-γIL-17 ratio in the CHB(1.13±0.85,P<0.01),the LC(1.69±0.92,P=0.010)and the PHC group(1.76±0.84,P=0.011)were all lower than that in healthy conlrols group(2.66±0.70),and the ratio in the chronic severe hepatitis B group(3.68±0.42)was higher than that in healthy controls group (P=0.004).IL-4/IL-17 ratio in the LC(0.72±0.38,P=0.026)and the PHC group(0.63±0.19,P<0.01)were both lower than that in heahhy controls(1.04±0.23).Conclusiolls The dominant expression of IL-4,IL-7 and IFN-γ is respectively found in the CHB patients,LC and PHC patients,and chronic severe hepatilis B patients.

ABSTRACT

Objective To investigate the relationship between the frequency of peripheral blood natural killer cells (NK) and HLA-Cw alleles in liver cirrhotic patients with chronic HBV infection and a-cute hepatitis B patients. Methods Thirty liver cirrhotic patients and 30 patients with acute hepatitis B were included in our study, and 41 healthy individuals were enrolled as controls. The numbers of circulating NK cells and activated NK ceils were analyzed by flow cytometry. HLA-Cw genotyping was conducted with polymerase chain reaction-sequence specific oligonucleotide (PCR-SSO). Results The numbers of circu-lating NK cells and activated NK cells in liver cirrhotic patients were 13.22% ± 4.61% and 45.68% ± 14.64%, which was lower than that in healthy subjects (P < 0.05). The numbers of circulating NK cells and activated NK cells in acute hepatitis B patients were 22.62% ± 3.70% and 65.28%± 14.45%, which was higher than that in healthy subjects(P < 0. 05). There were statistically significant differences between the two groups(P < 0.01). The allele frequency of HLA-Cw * 15 in the patients with cirrhosis was signifi-cantly higher than that in the healthy (P < 0.05), and there was a significant negative correlation between the frequency of HLA-Cw * 15 and the numbers of activated NK cells in liver cirrhosis(r =4). 862, P < 0.05). No statistically significance was found between the group of acute hepatitis B and healthy subjects a- bout HLA-Cw(P > 0. 05). Conclusion The function of NK cells in liver cirrhotic patients is low, HLA-Cw * 15 gene may be one of the causes of effecting the antiviral function of NK ceils to induce the persistence of hepatitis B virus(HBV) infection.

ABSTRACT

Objective To discuss the relationship between antibiotic prophylaxis and surgical site infection(SSI) in colorectal cancer.Methods A survey of 880 patients with colorectal cancer from Jan.1998 to Dec.2008 were studied.Patients were divided into two groups by antibiotic duration: ≥24 h group(n=401) and 0.05).Conclusion Longer operation time,more blood loss and longer bleeding time are the risk factors of surgical site infection.Moreover prolonged antibiotic prophylaxis can not decrease the incidence of SSI.

ABSTRACT

Objective To understand the distribution,variance and dynamics of Oncomelania snails in the water system of the Bianmin River after the river bank being protected with concrete.Methods The snails in the water system of the Bianmin River were investigated with the systemic sampling method and environmental spot-checking method in spring from 2002 to 2005.The indexes including area with snails,area with infected snails,rate of frames with snails,density of living snails and infection rate of snails in different environments were collected,analyzed and compared year by year.Results From 2002 to 2005,in the main river of the water system of the Bianmin River,irrigation areas and the adjacent marshlands of the Yangtze River,the areas with snails were 20.94-31.88,39.30-48.08 hm2 and 275.13-292.31 hm2,respectively;the rates of frames with snails were 9.96%-21.88%,29.61%-58.54% and 62.14%-85.97%,respectively;the mean densities of snails were 0.97-1.86,0.92-3.96 snails/0.1 m2 and 7.72-11.15 snails/0.1 m2.In 2005,in the main river,irrigation areas and the adjacent marshlands,the rates of frames with snails were 21.88%,39.33% and 83.81%,respectively;the densities of living snails were 1.30,3.96 snails/0.1 m2 and 11.15 snails/0.1 m2.During the last four years,the rates of frames with snails increased quickly in the main river of the water system of the Bianmin River;the densities of snails increased by 4 times in the irrigation areas;the indexes mentioned above were stable in the adjacent marshlands of the Yangtze River.However,the rate of frames with snails in the marshlands was 2-3 times higher than that in the irrigation areas and the mean density of snails in the marshlands was 3-8 times higher than that in the irrigation areas.Conclusions The main factor causing the increase of snails is that the river beach has not been cleaned yet in the water system of the Bianmin River,and so the key of schistosomiasis control is that first,the river beach should be cleaned completely,and at the same time,strengthening the snail control in the irrigation areas and the snail surveillance and snail control with molluscicides in the outfalls of the rivers of the water system of the Bianmin River to the Yangtze River.
